The flag is a dark blue field with a 1:1 white canton in the upper hoist side.
In the canton is the city arms. In the fly side of the flag are three golden
star. In the three stars contain the outlines of Kentucky, Tennessee, and
Virginia. This represents how Middlesboro is located at the tri-point of the
three states. The field color and the stars could represent space since
Middlesboro is located in a meteorite crater.

The city arms consist of a shield which contains some mountains and a blue
heraldic lion. Under the shield is a white banner that says "Pulchra Calles"
which translates to "Beautiful Streets". Above the seal is a traditional
heraldic knight's helmet with a blue and grey torse. Above the helmet is a
golden crown with the city and state name. Above the crown is another blue lion.
