The city emblem was adopted in Apr 1894. but there is no flag law. The blue flag charged with yellow city emblem
in the center. The flag proportions are 2:3.
The main device of the emblem is a Miotsukushi used for a beacon in the shallows of Osaka.
The prosperity of Osaka is greatly due to transportation by water. The Miotsukushi was chosen
for the emblem as it is deeply connected with its port.
Osaka uses a vertical city flag as well.

Because of the central "Red Cross" on this flag, this may be a possible WWII-era Japan "Medical Academy" flag.

It was a Red Cross auxiliary flag from Sumiyoshi Ward, Osaka. The texts read:
"Sumie-Nishi 6 Higashi Chome Red Cross Service Branch"
"Friendship Club".

Adding to Miles' comments, the flag is from "墨江" (English: Sumie), a
District (number 6) within the "住吉区" (English: Sumiyoshi Ward), part
of Osaka (City).
Again, as Miles points out, the inscription is as follows:
Top: "墨江西大東町赤十字奉仕分園"
Bottom: "親交會" (English: Friendship Association)
