I was told that the city hall of Louisville does not have a flag. However, a 
flag which looks like the one adopted in 1975 can be seen at
https://www.youtube.com/watch?v=P91W7a3BSSE. After receiving information 
from the historical museum (which does hold meetings there), I sadly didn't 
receive a response regarding my question as to what this flag is. For now, it is 
unconfirmed if the city uses a flag.

A Louisville flag was adopted in late 1975 in anticipation of the 
 Bicentennial and Louisville Centennial (hence the date 1878 - 1978). It was 
 designed by Barbara McKee and sewn by Loretta Jacoe. The intent for this flag 
 was to hang in the city council chamber for several months and hang in a 
 display case in a public library or historical museum if one were to be 
 established (which there is). I was told by the museum that it does not hang in 
 any of these places.
